I agree to purchase 3 therapy sessions (total of 4 hours) with Dr Tracie O’Keefe DCH, at a total cost of $1,100 AUD.

I understand that:

Full payment for the 3 sessions must be made in advance.

None of those sessions can be missed or substituted and I need to turn up for every session on time and make myself available for the full session.

If I do not turn up for the session, the session will be charged in full.

If I discontinue therapy for any reason or fail to show up for an appointment for a period of two months or miss two consecutive appointments, no refund is due and the contract is nil and void.

I understand and agree that AHEC offers no guarantees of results and that results vary according to the case and the amount of effort I, as the patient, put into the therapy, both within the sessions and outside the sessions.

I understand that I will be required to confirm the date and time of my appointments online at the end of each session and that there can be no rescheduling of the appointment.
